Online
You can enroll online. You’ll just need a few documents.
- Medicare card – the blue-and-white card with your Medicare number
- Certificate of Credible Coverage – for Part D prescription drug coverage; you will need to submit separately
- Notice of Replacement of Medicare Supplement Form (NOR) – you might need to submit this separately
When you are done, you will see further instructions for submitting documents separately.
Note: Current HealthMate for Medicare members will not need to submit these documents.
Download the Plan 65 Application, type directly into the form, and save. Print, sign, and mail to:
