Program: EQUIP HEALTH MEDICAL P.C.
Program Summary: Equip Health Medical PC (“Equip”) will deliver virtual outpatient care for eating disorders (“EDs”) by providing Subscribers with a 5-person Care Team: therapist, dietitian, peer mentor, family mentor, and medical provider that will service subscribers and their families using a secure telehealth platform to deliver both unlimited video-sessions, between-session messaging, a shared medical record, and weekly treatment team meetings. Treatment is based on a cohesive team-based approach that is family-focused, goal-oriented, and evidence-based. Equip will screen for medical instability, acute suicidality, and psychosis, and provide a referral to a higher level of care if needed.
PHASE 1 (months 1-4): Intensive Services - Unlimited video sessions with Equip multidisciplinary care team – therapist, dietitian, peer mentor, family mentor, psychiatrist (at least weekly with therapist, peer mentor and family mentor). Care Team members are Equip employees who have been trained in evidence-based treatment for eating disorders, and receive weekly supervision, case consultation and ongoing training. Patients and families have access to virtual support groups and ED resources and are encouraged to communicate with their providers outside of session via messaging. Care Team members have access to a strong community of evidence-based ED experts to share learnings and continue to advance their treatment knowledge and skills. Care Team members track symptoms and update treatment plan accordingly on a weekly basis.
PHASE 2 (months 5-12): Holistic Services - Equip will slowly ramp down clinical support while keeping weekly peer support sessions in place, plan for relapse prevention, and have adolescent eating 100% independently for two (2) months before discharge. Equip anticipates at least weekly sessions with peer and family mentors, biweekly sessions with therapist, and monthly sessions with physician and dietitian. Subscribers who need clinical care ramped up will receive unlimited services, as required. If weight drops or behaviors return, Equip will return to weekly sessions with Care Team providers, but the Participant shall remain in Phase 2.
PHASE 3 (months 13-24): Relapse Prevention Services - Equip will provide continued access to the Subscriber’s full Equip care team beyond month 12. Some patients may require ongoing treatment for up to an additional twelve (12) months, specifically designed for relapse prevention and treating co-occurring illnesses. Phase 3 will typically consist of continued sessions with peer mentors, individual therapy with a therapist, medication management with a psychiatrist, and ongoing access to group classes and Equip’s resource library. Subscribers who need clinical care ramped up will receive unlimited services, as required.
