Please note Avastin (J9035) does require Prior Authorization to be reviewed by Prime for Medicare members. This has been in place since January 1, 2023, but was inadvertently left off the Prior Authorization for Drugs list. If you have a claim that is denying, please contact the Provider Call Center to have the claim reprocessed.
