Many providers fail to complete the practitioner change form when closing a location or leaving a practice. It is crucial to make sure we have your current practicing addresses in the Provider Directory, as well as removing your old or closed addresses. We want to ensure the best quality data and information for our members and for compliance measures with the Centers for Medicare and Medicaid Services (CMS).
