Plan 65 options for HealthMate for Medicare (PPO) members
HealthMate for Medicare (PPO) will be discontinued after December 31, 2024, so you will need to choose a new plan for 2025.
This page shows the Plan 65 choices that could be a good fit for you and your health needs. These Medicare Supplement plans:
- Offer limited out-of-pocket expenses, covering most deductibles and copays for Medicare Parts A and B (hospital and medical)
- Include nationwide coverage and no referrals; you can see any provider/hospital accepting Medicare
- Don’t cover dental, Rx (Part D), vision, or hearing; you would need to purchase separate coverage

NOTE: If you don’t take action before December 31, 2024, you will lose your prescription drug coverage and only be covered by Original Medicare (Parts A and B—hospital and medical) starting January 1, 2025. Medicare requires you to have "creditable coverage" for Part D drugs.
